Job Description:
A company is looking for a Senior Care Manager (RN).
Key Responsibilities
Develops and assesses ongoing care plans/service plans and
collaborates with providers to meet member needs
Coordinates care between members, families, and care provider teams
to ensure person-centered services
Monitors member status and care plan goals, revising plans as
necessary in collaboration with members and providers
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
4 - 6 years of related experience
New York State RN license required
Pediatric population experience preferred
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
